Your Role
At Plume we envision a member experience that provides thoughtful, expert, timely, and gender affirming care that celebrates and enhances the quality of life for every trans person.
As a hybrid Care Coordinator, you will be the first point of contact for patients in Florida, ensuring high levels of patient care and satisfaction in a remote, telehealth environment with in-person shifts throughout the month. Reporting to our Care Coordinator Manager, your responsibilities will include answering prospective and current patient questions, triaging inquiries to the appropriate medical staff, ensuring proper data flows, and helping support the lab, pharmacy and other resource needs of our members. This is a hybrid role based in Florida. We are looking for someone who can commute to various in-person clinics throughout the state of Florida, part-time. The role also requires remote work when not at an in-person clinic. We are offering a full-time, non-exempt role with competitive pay and benefits.

Must-Haves

  • Deep understanding and compassion for the trans experience
  • Excellent customer, patient, or member service with 2+ years in a customer service role
  • Attention-to-detail with strong data management and entry skills
  • Respectful and empathetic written and verbal communication skills
  • Understanding of basic computer skills, learning new computer systems, and interfacing with patients and colleagues in a remote, digital environment
  • Hybrid, in-person care coordination
  • Ability to work at least one weekend (Sat or Sun) per week
  • Ability to travel up to 50% of the time throughout the state of Florida. Note - time paid will be travel time to and from the out-of-town clinic, and work time, either at the clinic or at your home office.
